Transitional Care and Why It May Be the Best Option

After hospitalization, many seniors have difficulty switching back to normal life without the support regularly received by the caregivers in the hospital. Many return to the hospital shortly after discharge because they didn't receive proper care once they were home.

The majority of seniors need a period of in-home health care to transition them back to independent living. They should cover daily tasks like bathing, dressing, grocery shopping, light housekeeping, ambulation assistance, medicine reminders, and daily grooming. These seemingly simple activities can make a huge difference in a successful recovery and return to independent living.

When a senior is released from a hospital or nursing home, the industry standard for readmission is 20-25%. When they come home with home care services the chances of a readmission are significantly less. We want to make sure that every mom and dad gets the proper care they need while being comfortable in their own home.
