When your mom or dad have to undergo surgery or suffered from an injury, they should be given the compassionate assistance after major surgery, a serious accident, or short-term disability. At Golden Care, we provide transitional care services to help families facing the need for in-home senior care in Collier, Lee and nearby counties to avoid the emotional struggles of providing daily in-home care for their parents.

We understand the first few days at home following discharge are most critical to ensure healing and wellness. To ensure a
successful transition from the hospital or rehab to home setting, Golden Care will assess the needs of the client and provide
an individualized plan of care. From just a couple hours after leaving the hospital to a 7-day transition plan, we can work around your needs and budget to ensure a smooth transition to your home.

Transitional Care Services
Transitional Care and Why It May Be the Best Option

After hospitalization, many seniors have difficulty switching back to normal life without the support regularly received by the caregivers in the hospital. Many return to the hospital shortly after discharge because they didn't receive proper care once they were home.

The majority of seniors need a period of in-home health care to transition them back to independent living. They should cover daily tasks like bathing, dressing, grocery shopping, light housekeeping, ambulation assistance, medicine reminders, and daily grooming. These seemingly simple activities can make a huge difference in a successful recovery and return to independent living.

When a senior is released from a hospital or nursing home, the industry standard for readmission is 20-25%. When they come home with home care services the chances of a readmission are significantly less. We want to make sure that every mom and dad gets the proper care they need while being comfortable in their own home.

Our carefully trained caregivers can provide a full array of personalized homemaker & companion care services that may include, but are not limited to:
  • Assist with packing client with belongings from hospital or rehab
